If there is no answer, please leave a voice mail message. Calls will be returned within 24 hours, Monday-Thursday. Calls received after Thursday at noon or on a holiday will be returned the following Monday or the next business day, as applicable.

Effective December 1, 2022, Dr. Sloan no longer sees pain patients in an office setting. His practice is now limited to providing anesthesia services in a facility setting. If you are a past patient of Dr. Sloan’s and need your medical records, please call the office at the number below (do not send an email unless you also call the office). You will be required to sign an authorization form authorizing Dr. Sloan to deliver your medical records to the person designated in the signed form. The form can be downloaded from this website via the green button below, or you can call the office and get the form.  Please submit the completed and signed form to office@mattsloanmd.com and call the office to let us know to expect it. Requests will be processed in the order received.  Please allow 3-5 business days to process your request. Electronic medical records can be sent to other physicians at no charge. Otherwise, the following fees will apply:

1-20 Pages:  $25.00

Over 20 pages: $0.50 per page

Payment must be made before records will be released.
